Player with arthritis here, and some of your post makes me worry that you're a prime candidate for arthritis and/or carpal tunnel down the line. I don't want that to happen to anyone if it can be avoided!
I'm glad you're listening to your pain and adjusting the way you play, but I'd also recommend taking a 5 minute break every hour and periodically doing stretches. Trust me, you really don't want to get to the point where you develop a condition, and taking care of your hands will help you steer clear of that. So I really recommend being proactive now to prevent ending up in a place where pain is just part of your daily life. As far as controllers and hotbars go, I have a few tricks that have helped me to be able to play comfortably, so maybe they'll help for you as well?
MacrosMacros are wildly misunderstood, which is understandable because the game doesn't explain them and that has lead to players unwittingly spreading a lot of misinformation about them. But if you understand macros, they can enhance your play while helping you take care of your hands. I use these to display the icons of the abilities that aren't otherwise displayed on my screen; for me, that means the abilities that are available when I hold R2+L2, as well as any abilities that are activated by macros without being the visible icon for a macro. I arrange the icons in consistent ways across my various jobs, such that there's a basic pattern of icons in certain places relating to certain buttons on my controller.Now that I take breaks, stretch, and use macros and action trays, I've found that I can generally play FF14 both effectively and comfortably.
