Who really cares?

Well adjusted people, and presumably developers of popular MMOs that don't want portaits of catgirls giving head to run them afoul of the ESRB.

You only see the adventure plate if you click on someone and look at them. You can disable portraits if you dont want to ever see anything suggestive.

"I shouldn't have to stop making juvenile porn portraits, other people should just lock themselves out of an entire feature of the game."

Grow up.

I just dont see how adventure plate/portrait lewdness is so so much worse than actual story themes or players glamming themselves to look as sexy as they can.

One day you'll grow out of your teens and it'll make more sense.
