The friendlist has not been updated since 1.0 ARR.
Every big MMO has updated their friendlist system, but not Final Fantasy XIV.
There are huge problems with the current Friend List system which create more problems for customer support and also make players quit the game due to being stalked or griefed by people who were once their friend but are no longer. Being friends with someone should always be mutual.. never one sided which is not the case in FFXIV. You can be on someones friendlist even when you have removed them from yours. They can then stalk you and track your location knowing what city you're in what datacenter you're on at all times. This makes it very easy for a stalker to follow you everywhere. Making people very uncomfortable to the point they do not want to login to play the game. I have several friends who are followed around in-game by people they once had on their friendlist which has caused them to not want to play or even quit entirely.. and I am positive many more players have experienced the same. Here are some changes to the current system that would make it better for everyone.
#1 - Make being a persons friend always MUTUAL - This means that if I remove someone they should also no longer have me on their friendlist so they cannot stalk, track, or grief me creating problems for customer support and I should know if someone is my friend or not. We shouldn't have to question who are friends are. We should just know.
#2 - Uncap the friendlist limit - I shouldn't have to delete friends to be friends with a new player that I want to be friends with. This makes other friends very sad when they find out you have deleted them for someone else which could make them quit the game due to sadness.. I'm being serious.
#3 - Give us an invisible mode - Sometimes people just want to be left alone even by their friends but still want to play the game.. /busy mode is not enough.. people can still find where you are and come follow you around and bother you despite you wanting to be left alone at that time.
