    (I'm not sure these details are accurate, but I believe that was the attack that occurred just before when, out of nowhere, the achievement was unlocked for me. My party members can verify that the achievement unlock message showed up in the chat. I believe this occurred just after the first instance of Pandaemoniac Ray used during that specific encounter)

    This is most likely because hidden enemies are spawned during many raids who carry out mechanics (such as area attacks) but you can't attack them back. They can be considered enemies by the game so when they are removed after the mechanic is over, it's possible it thinks you "defeated" them.

    Is that how mechanics work? No wonder many of them are so janky, then.

    Just certain mechanics, mostly in raids and especially in extreme and savage. A standard dungeon boss with some aoes isn't usually done like that.
