This is as old as the MMO genre itself, frankly all of the issues you raise are. I still remember the mass rofls and ridicule that flew after a key officer and raider in a rival guild accidentally typo'd his e-girl's cyber into /say at a contested camp. RO's 3 way love triangle on Bristlebane was worthy of a documentary if it had have happened today. The list goes on. Crazy times.
Simply making it against the ToS isn't really a viable answer though, there's a reason no ones done it. Obviously there's the money side of things, MMOs are glorified chat rooms to many, always have been and always will be, remove that aspect of things and you take away the soul of the game (note how lifeless solo queue DF usually feels when no one says a peep?). The other aspect that's routinely overlooked is actually policing it. The GM staff is clearly barebones as it is. How do you expect them to police this sort of stuff fairly, effectively and efficiently? And how long do you expect GMs to tolerate having to deal with it for before they pack their bags and run for the hills?
