Part 1: Naming conventions and culture
Basic name structure
Female: [Given name] [Family name]
Male: [Given name][apostrophe][suffix] [Family name]
An epithet can be used instead of a family name. (Female example only: Yabi Two-tails)
Keeper names are not gendered in the sense of requiring separate lists of male and female names, because male names are effectively "Xth son of [mother's name]", with their given name being the same as their mother's name and the X being indicated by the suffix. Consequently, brothers will have the same given name, only differentiated by suffix. (Lower number suffixes are of course much more common, rarely going above 3.)

The suffix is potentially optional, as we have one example of a male Keeper using a shortened name without the suffix – Nhagi'a Jakkya, who prefers to be called Nhagi – so it seems likely that they themselves regard the names as unisex rather than feminine.
However, it is likely that only the eldest (or at least one per family) son would use the shortened name without suffix, due to the rigid format of male names. To continue with the example of Nhagi'a Jakkya, if he had a younger brother his name would be Nhagi'to Jakkya, and it is unlikely that both would be called Nhagi for short. We have no canon examples of how this might be handled.
Use of surnames
For the purposes of character creation, surnames are simple: just put it in the surname field.
However, lore-wise they are treated as somewhat more important than just a surname. Where NPCs of most races will only have their given name displayed overhead and on speech bubbles, Keepers are labelled with their entire name.
In conversation they are likely to just be addressed by forename.
Basic culture
Keepers are one of the less explored cultures of Eorzea. We know that they are local to the Black Shroud and not always treated as welcome in Gridania, and that there is a population of poachers based in South Shroud. Traditionally they live in small groups of two or three families, with a family unit consisting of a mother and her children and possibly other adult female relatives.
It is implied that the men traditionally keep mostly to themselves as lone hunters, only occasionally coming together with others, and are not regarded as a permanent part of a family. It is indicated to be culturally normal for both genders to have multiple partners over time.

- The Rising 2023 – "Siblings Rising" – Kipih Jakkya learns that her brother Nhagi (sixteen years old and a new adventurer) has been behaving recklessly and putting himself in danger, so she revokes her consent to him being an adventurer and orders him to return to their village and start over. It is later noted that she has raised Nhagi since their parents died six years ago, so it seems this order is more as mother than sister. (Once again the FFXIV writers can't keep track of their lore-established unconventional family setups though. You could spin unique circumstances for them, but it's still annoying.)
They also mention having an aunt who lives "in the mountains".
Part 2: Name lists
NOTE: There are three names I am not 100% confident of including.
One is a 1.0 character called "Flame Private Dhavha" but it is not certain whether this is meant to be a Seeker or Keeper character name.
Another character is a 1.0 enemy named "Xha Viqqoh" which sounds very much like a Keeper name but the GE wiki has no screenshots and only lists her as "humanoid".
The third is a pair of names from the name generator: Bathu'a and Bathus. This is the only example of a male name not matching a female name. Both seem a bit strange.

Part 3: Name lists expanded - the Mithra
Mithra are a playable race in FFXI and essentially close precursors to the Miqo'te.
Their names strongly resemble and sometimes overlap with Keeper names, therefore can potentially be used as an expanded set of name options.
The Keeper name lists are fairly limited, with only a small number of characters and few of them venturing outside of the lists from the character creator, so adding Mithra names greatly widens the pool of options.
That said, if you want a lore-accurate name then it's probably better to stick to the limited number of surnames that are confirmed Keeper surnames, but there is likely more scope for variety with forenames.
Note: to my non-FFXI-playing awareness, all encountered Mithra are female with the sole exception of Lehko Habhoka, whose name doesn't look particularly different to the female names.
The Vana'diel Tribune (official newsletter framed as an in-universe newspaper) ran a series of articles on race naming conventions. Relevant parts from the article that covered Mithra names:
The Mithra are a very clever race, making them excellent hunters. But what are their names like?
Examples include "An Shanaa," "Hanaa Punaa," and "Fo Mocorho."
While out hunting, they call each other by only the first part of the name, "An," "Hanaa," and "Fo" would be used in the examples given. In fact, they seem to do this whenever they are closing in on something or in a tense situation.

There are two additional Mithra characters named Yahliq and Zshahda which I have not included in the list as I think they are likely to be Near Eastern names -- they have no surnames and are in contexts where they might have been given local names over race-conventional ones. Yahliq is a ship merchant on a route to Al Zahbi and Zshahda is from a quest where one of the other NPCs is a Tarutaru named Bhjaama, which I believe is the only example of a Tarutaru character not following their standard naming conventions.

Name sources
I took the list of character creator names from this Google doc listing all names from the character creator.
Gamer Escape also provides names from the character creator (not alphabetical), NPC names and the FFXI section of the wiki has a list of Mithra NPCs.
Update history
September 2023: Added notes on the Rising quests. Added Raghu'to Zhwan to character list and consequently Raghu as a name option. List of character locations in progress.
June 2024: Discovered some Keeper names were missing from the combined Keeper/Mithra list, so rather than cross-checking each one, I have replaced the whole list with a newly merged version.
February 2025: Went through Gamer Escape's full list of FFXI NPCs and discovered some additional characters that had been missed from the list of Mithra NPCs:
- four new NPC names: Bebo Jhatabia, Fhu Madihmin, Yai (no surname), Zshahda (though I think this is a Near Eastern name -- see note in the Mithra list above)
- names associated with face options in the character creator: Opih, Pehl, Shamih, Suh, Yuhn, Cyahl, Myo, Soih
- bonus: some names that Mithra characters gave to their pets: Azo, Kyo, Pya, Vahi
- I also realised I spelt Lehko Habhoka's name incorrectly and have now fixed it.
March 2025: - found some additional Mithra names in the blurbs and short stories for seasonal events: Cha Shya, Fhara Mashuhba, Payuh Milmorah, Rhe Tei, Sheh Teela, Yah Teih, Yutte Jaborah
- another probably-Near Eastern name: Yahliq
