So has Confiteor and the blade combo always been just damage, sacred chain and no healing? Because I could’ve swore it healed you, but when I looked it doesn’t. Has it always been this way, or did I just assume it was because of PVE?

I assume youre talking about the royal authority combo, or do you mean Phalanx either way Once a target is hit with confetti, they get a debuff called sacred claim which heals you or your allies 2k per attack, you should be able to see it in the battle/attack log(or whatever its called) if you're not seeing it or if it's not happening then might be a bug but tbh the healing is a bit minimal so maybe its just not noticeable
It is PvE where Confiteor and Blade of Valor combo do healing to the Paladin directly. In PvP this is revised as a utility benefitting your whole party but requires additional attacks.
Upside, oGCDs (Abilities) like Shieldbash and Intervene count for the 2000 HP heal and an AOE counts for each Sacred Claim target hit, so a Warrior or Dancer with their spin dance gets a lot of value from it. It also stacks on top of your Atonement heals.
The Atonement heal probably was made like PvE attack spells healing due to frequency of them but still being melee range
