It's worrisome but I'm also curious if the game's age has anything to do with it.

Probably does, the bigger and older a game grows the more flaws at it's core get exposed.

Destiny 2 is running into the same issue where the game becomes buggier with every year, but XIV didn't have the most refined engine to begin with.
