I have added these testings done to still try to fix this issue at the start of this thread. So far, error has usually been either in Limsa or when doing 8-MAN TOP ultimate. It was so painful we wipe mid pull during P3 HW phase.
Changed to DX9
Changed to Windowed mode
Bought a new Monitor as the other old was 165hz + 144hz, now it's 144 hz + 144hz dual
Hardware diagnosis and deep clean PC (no issues here) - note that I can play recent games on ultra 1080p 60fps
Halved the refresh rate in game from 144hz to 72hz (FFXIV System Configuration > Display Settings > Frame Rate) as advised here https://forum.square-enix.com/ffxiv/...THIS%21/page35
Set full-performance mode in NVidia Control Panel->3D Settings->Global parameters ->Power management
Uninstalled Citrix Workspace (PC is being used for both work and leisure)
All Steam game apps mentioned in this testing were cleaned as I have reformatted my PC countless times already to no avail
Deleted FFXIV local files
Monitored extensively Event Viewer but have not seen any error or warning in relation to the DX errors (AT ALL)

Okay something weird happened while in DX9. It didn't crash, yet. But after an hour or so AFKing in Limsa, FPS dropped horrendously like 1-3FPS (powerpoint slideshow movement). No background apps or desktop browser opened. Maybe this is related to the DX error?

We are sorry to hear that you are still experiencing these issues. To further troubleshoot, we would suggest that you try lowering the refresh rate on your monitors. Additionally, we would suggest that you try using only one monitor at this time as you mentioned using dual monitors. This would be just to see if this helps with the issue you are experiencing.

However, I've done these tests and have encountered errors still.
I played Cyberpunk 2077 just now and used all settings to Ultra with Ray Tracing and have no issues playing for about an hour or so. I also tried stress testing my CPU and GPU with 3DMark but have resulted that there were no issues as well (both were OK without overclocking).
Issues encountered to be randomly getting the errors in Limsa (both under windowed or on full settings; dx 9 / dx 11; and on lower refresh rates as well) and during raid in TOP.
For Limsa, I just AFK there as this is usually the most crowded area to replicate the error. It does not happen abruptly, but it does happen after randomly of AFKing or so. Sometimes longer than an hour, sometimes shorter.
For TOP Ultimate, it usually happens at logging into the instance after playing for quite awhile (4 times), during Phase 1 around Program Loop (wasn't able to count but it probably around 5) and in Phase 3 (Intermission to Hello World first towers). Also, the errors don't come outright during TOP. Every animation stops but I can still move freely around the arena even on the dead zone. My static even comedically exclaims that my dead body keeps on moving around. This happens for about 10 or so seconds and then I get the DX / launcher error.
I suspect these are memory leak issues as this are usually the symptoms of those (playing for awhile, and then everything goes wrong after). But again, all of these are conjecture as there are no definitive causes.
