Hi, I was wondering how you become a mentor?

I currently have a lvl 27 Tank, 27 healer, 30 DPS, and I will be working on each class eventually. I know these levels are not high enough to become a mentor, but I was wondering what are the requirements for when I want to mentor later.

Is there a certain amount of classes you need at 60? A form to fill out? etc.
