Stealth didn't move to the action bar

Hi,
I would like to report a bug that is currently affecting my character.

Character Name: Dionysus Ludlow
World: Excalibur
Date & Time: It happened once when I've reached lv28 with my miner class, and it's currently persistent.
Frequency: persistent
Race: Hyur / Midlander Male
Class: Miner
Area and Coordinates: Humblehearth
Party or Solo: Solo
In-Game Time: -
Steps:
1.I've Reached lv28 with my miner class. Usually new actions appear on the action bar, and then I start accomodating them according to my needs, but that time Stealth 3 didn't appeared on the bar.
2.Tried to add Stealth III to my action bar, the action changed color like when one becomes available, but is not underlined and clicking on it doesn't move it on the bar (although it would be not necessary doing it)
3.I'm not able to use it...

It may help: when I've reached lv28 I was under the effect of Stealth II.

Am I missing something?
I can provide a screenshot if it may help...currently I'm not able to use Stealth III.
