Greetings bloodmagick,

We're very sorry to hear that this issue is still occurring. It does appear that you've tried numerous troubleshooting steps already to try and resolve this issue. However, I would recommend giving the following a try to see if they might help with these crashes:

- Perform a clean boot by opening the Task Manager and selecting the 'Start up' Tab. Once there, please disable everything listed, and restart the PC. This will ensure that only the necessary programs start-up and run when booting up after the restart to ensure that nothing could possibly be interrupting the game from running.
- Make sure that the Windows OS is fully updated (including any optional updates, as they are not usually automatically installed).
- Turn off Fullscreen Optimizations. Find the executable file for the game (normally found in the install folder), right-click it and select 'Properties'. Under the 'Compatibility' tab, check the box labeled 'Disable Fullscreen optimizations'
and then click 'Apply'.
- Make sure your anti-virus software has the appropriate exceptions for FINAL FANTASY XIV (or you can temporarily disable/turn off the program for testing purposes).
- Try to remove local files that could also possibly be corrupted. You may do this by going into 'My Documents' > 'My Games' > 'Final Fantasy XIV: A Realm Reborn' and delete that folder or possibly rename and move it. (Please note that this folder contains UI element settings, Macros and screenshots, should you not wish to lose them and possibly re-use the settings, you may opt to just rename and move the folder rather than deleting it)

I hope this helps! Thank you for posting in the FINAL FANTASY XIV Technical Support Forums.
