I think an achievement at 10K would be nice, but make it a title, not a physical object. "The Revered" might work.

This is just going to encourage people to spend even more time farming commendations in guildhest roulette, and dragging everyone else back to level 10 with them.
What for? It's just bragging rights at this point.
They could put an orchestrion roll achievement instead. Since some of the rolls cannot be purchased directly on the market board, some have to be purchased with bicolor gemstones. It would have to be a really high count for the achievement though, like pretty much all the rolls up to 7.0 that can be obtained in-game. Would have to not count those from the mogstation or promotions.
I definitely think they should add a lot more. One for 5k, 10k, 20k 50k and 100k. And then wait for people to post on the forums that mentors need at least 100k comms, because I know it'll happen.
They could add titles on 10k, 50k and 100k and add a floating commendation I medal minion for 100k. Or a mount. But none of this is important just a nice goal.
Commendations are only occasionally tied to good (or above average) gameplay. DPS are shortchanged most of the time. Healers and tanks just get tons of them. But I see it both ways. It is hard to stand out as a DPS in the first place. Secondly, how often are you waiting on content because of DPS? Aside from maybe the post-ARR 24-man raids, it is universally waiting on healers and sometimes tanks. Regardless of how well they play, we rain down commendations on them simply because we couldn't even get into that content without them.
By the OP's same token, "most" players have High Roller. Let's go ahead and make one for 2M Jumbo Cactpot and 10000 Triad Roulettes.

People still give commendations? Even as a sprout, I rarely get any nowadays, even if I play well and/or I'm tanking.
I guess I should start leveling my healers more.
There have been times when I busted my ass, carried a run, or just generally did extremely well and never got any commendations. Then other times I can die a bunch and get a couple. It has nothing to do with skill or playtime, it's just luck. I say this as someone who's played since the very beginning of ARR and I'm still about a stone's throw away from getting it. Then again, I don't beg with macros and stand at the exit while emoting, hoping people will give me one, so maybe my acquisition of them is a bit more organic.
I ask people not to commend me because I already have too many. I can be a mentor, but I'd rather just help people by myself.
Not sure where I am and can't check cause maint, but I never overly worried about those. They're not something I can control. And also don't mean much to me. I can do as healer what I consider is not really any extreme amount better than the others and get buried in comms or be the key to rescuing the entire run and get no comms. Even if they added a title or something, it's not something I would chase. People comm for their own reasons and rewards for something like this only work if they either make them required before you can leave or incentivize players to give them.
