    It's worth noting that the spellings of IRL family names can drift a bit over time as well, so there's a potential in-universe excuse for what is likely just a typo in the Romanized text. I have relatives with (to me) really weird spellings of what is technically the same family name when you go back far enough.

    As for game names... W'fharl (corrupted from my 1.0 Hyur name (also my forum handle), with tribe letter added) is close enough to the in-game phonetics ('falkh/'fhul) that it's not too out-of-place, while also not being something that would be pulled from the generator. Leaving that more as an example for posterity than for OP, as the question is now a month old.

    Definitely, and it's clearly going on for Seekers too. Looking through the compiled name lists turns up a lot of minor variations in similar names, which might be deliberate or accidental on the part of the writers. Letters vary and H's get thrown in or left out anywhere they feel like it.

    The naming conventions for Xaela has a whole thing on how their language is mostly spoken rather than written, and lots of variant spellings come up as a result, so I assume the same applies to Miqo'te shifting from their traditional tribe lifestyle to the cities.
