Having been running some levelling dungeons trying to get gear pieces, I am suspecting that the loot tables for all dungeon chests have been shuffled around and levelling dungeons are giving less gear overall as a result.
This needs more investigating but it is my current hunch. Unfortunately it is somewhat anecdotal and unconfirmable, as I can't wind back to see what it was previously doing.
I thought the levelling dungeons were previously a bit more generous and less structured with which gear came from which coffers, while the endgame dungeons followed a strict "shoes or belt from boss 1, hat or gloves from boss 2, shirt or pants from boss 3" pattern, plus (I think) a guaranteed accessory in each chest. Then, as I noted in my older post above, they took away belts and turned the first boss from "shoes or belt" to "shoes or equal chance of nothing at all" and you only get the accessory.
This seems to be the case in levelling dungeons as well, and I feel like the non-boss chests are dropping gear less reliably than they used to. I thought they used to always have one gear piece, but now it seems to have gone to only a chance, as I believe has been the system for endgame dungeons since at least level 70 and now seeming to have been applied back to the level 50/60 dungeons.
I may be jumping to conclusions but I wanted to make note of it. Certainly the first boss is often only giving an accessory and no gear.
