Crafted Item Disappeared

I've almost exclusively been doing levequests in Old Sharlayan the past few days and I've noticed occasionally a high quality item that I crafted disappears from my inventory. Usually I craft 3-6 items before turning them in. It occurred yesterday but I wasn't paying careful attention so I chocked it up to crafting less than I thought. It happened today again when doing levequest on my blacksmith. I'm certain I crafted 3 High Durium Tathlums. The chat box shows 3 crafted. When I turned in the quest, I only had 2 in my inventory. I did a search to see if the missing one stored elsewhere but its nowhere to be found. I'm still kind of a noob with the game. My inventory was not full. Is this a known problem? Any suggestions?
