That's probably your graphics card dying.
If your drivers aren't up to date, do that. Check your temperatures it could be overheating, may need a good dusting or perhaps underclocking if it's that far gone.
Or it could be too late. Best of luck. Thankfully graphic card prices are starting to drop...
