not sure how the third eye item would really fit into the lore though, since pure garleans with the third eye use the hyur and elezen (with hyur ears) models. which also makes making them their own race kinda awkward too

It could just be an item called "False Garlean Eye" and it would work. I can see them doing that as one of their fashion accessory items too, like the umbrellas and the glasses.
It is super awkward that the Garleans never got their own race. Back in 1.0, people in my linkshell and I even debated on whether or not Van Darnus was secretly an Elezen because she used Elezen motions. It would've been one thing to consistently use Elezen and slap a third eye and some Hyur ears on them, but there are Midlander sized Purebloods out there too like Cid, Midas, Jenomis, and Jullus. Their lore isn't even always straight because Arenvald said once that half-Garleans don't get third eyes but the half-Garlean in the Weapon quests has one, though it's black instead of white.
But I don't see us getting a Garlean playable race for the number one fact that they aren't even their own race in the code of the game more than anything else, plus Yoshi-P said we're done after fem Hroth.
People I know who do it cling to the Resonant or the one line in the EE that says "Garleans have little inborn ability to manipulate their body's aether, however, and thus the arcane arts have ever eluded all but a few" and they consider themselves one of the "few".How does one RP as a Garlean while remaining true to the lore? It's easy for me to RP as a Lunarian because while they do exist in the world of Final Fantasy, there is little to no mention of them in FFXIV, so there are no lore restrictions when it comes to how I develop her story in regards to her heritage. Garleans also have a unique skillset that would almost make more sense if it was a job more than a new race.
Don't get me wrong, I'm not dismissing it. Just curious how it would work.

It would still be a little awkward to have playable Garleans given that the few non-antagonistic Garleans we see in Eorzea make it a note to keep their third eye hidden for rather justified reasons.
Cid is the only one who's particularly open about his heritage since he has a strong enough reputation that nobody questions it, whereas if some Garlean "adventurer" shows up out of the blue, it wouldn't seem terribly logical for nobody to bat an eye at it.
I would, however, like an Ilsabard-focused expansion partly for the sake of making Garlean-styled armor available a logical choice thematically.

I was going to petition to have Jullus join the Scions, and well... nvm.

It's pretty easy, especially with the Reaper quests now confirming that bands of Garleans can easily operate within the shadows across Eorzea. Others - such as myself - simply had our Garlean characters spend a lot of their time in Kugane, since Hingashi not only has Garleans living and working within the city but is neutral on the world stage. How does one RP as a Garlean while remaining true to the lore? As for the logistics, most people who role-play a Garlean simply go with a Midlander, Highlander or Elezen model and either hide the forehead with certain hairstyles or make use of a circlet to 'hide' the third eye.
A proper third eye glamour would be wonderful and allow people to make use of a broader range of hairstyles as a consequence.

just add garleans as a subrace for hyur and elezen. just throw a line in somewhere that hydaelyn gave you the ability to use aether normally because of the whole paradox from elpis (we receive the blessing from hydaelyn -> we go to elpis and meet hydaelyn -> she gives you the blessing because you would have become tempered to ifrit and killed and therefore unable to have met hydaelyn in the first place)
have the blessing include that. or just have us be extra special and allow it due to us being a shard of azem

One thing I've always wondered is if that's really an undeveloped eye kind of like on blind cave fish, or a little ball of bone/keratin.

Why is your default asking for it to be in the cash shop?
