I assume proper citizens can come and go from the city whenever they please, they just usually don't because they're comfortable in decadence. I mean, Ran'jit and Vauthry leave Eulmore to visit Lakeland during the story.

Also, Tristol just got lucky he didn't get turned into Soylent green.

Free citizens can come and go as they please, they just rarely bother to leave because most of them are complacently wallowing in their debauchery. It's not that nobody CAN leave Eulmore, it's that they don't want to.
