The rules language specifically says "Leaving the *game*" or "Disconnecting." That language is important, because they do use the term "instance" in other parts of the ToS.
Leaving an instance does not equate to leaving the game. It is encouraged to leave instances if playstyles disagree, you find yourself unable to play, etc etc. What is not encouraged, and is specifically disallowed, is leaving the game. This is because when leaving the game, you sidestep the penalties of leaving an instance. Your character remains in the instance for some time if you leave the game, and if it's in the beginning of an instance, your party members would have to wait 5 minutes in order to remove you from the party and search for someone who wishes to join. Leaving the game can be done in a myriad of ways, but the most common is using the task manager or disconnecting your internet.
