    I think you're forgetting some major parts here.

    Nanamo's power is limited by the Syndicate, which is a group of the wealthiest individuals (and in Ul'dah, wealth equals power). She was essentially a puppet to them.
    Said individuals also weren't too happy about Nanamo's attempts on dissolving the Syndicate, with the help of the other two city-states (Gridania and Limsa Lominsa).
    Teledji was especially unhappy about this, and made an attempt to assassinate the Sultana. An attempt that Lolorito had learned of. While he wanted Nanamo to be removed from power as well (along with Raubahn), he had no interest in doing so through killing her.
    Why he didn't apply the antidote sooner? Because this is Lolorito we're talking about. If he thinks he can profit off of something (in this case, holding onto the antidote), he'll do it. Lolorito isn't some goody two shoes character, he's still a Syndicate member focused on wealth (because, again, wealth equals power in Ul'dah).

    But if there are no consequences why don't the Syndicate just take over? Clearly the Sultana can't stop them. Or punish them for anything they do. The Syndicate could apparently drug her, frame her allies for crimes, and do whatever the hell they want 24/7 without ever having to fear punishment.

    If they cannot be held accountable for anything they do, why even bother keeping the Sultana in power? What is she gonna do if they attempt to kill her and take over? Cause what I have seen is if they did so, her only option would be to ask them nicely not to cause she lacks the power to do ANYTHING.

    The presence of a Sultan or Sultanna creates a degree of stability within Ul'dah. If that is removed then the syndicate can and will try to get in that power gap regardless of the consequences. Most of the syndicate are aware of this, though Teledji was going to do so anyway, hence Lolorito's actions.

    And part of why Lolorito is able to get away with it is because the instant they charge him, he'll bring up that Raubhan executed a man without trial and that the Scion's resisted arrest and fled from authorites. Both claims that no one can dispute because they are verifiably true.

    Something you have to remember: Outside of Teledji, none of the Syndicate actually want greater power, because that comes with greater responsibilities. They largely just want to make fat stacks, because ultimately governing as part of the Syndicate is also just a means for them to make more money.

    Lolorito saves Nanamo's life not because he's a staunch royalist or anything; he saves her because he profits pretty well off the current status quo, and so wants to protect that status quo. And all the other living Syndicate members (outside of Raubahn, who is indeed royalist) agree; don't fix what's not broken.
