If it happens all the time, its not bad luck, its bad programming that makes the RNG very dependant on hardware, so some people will have "bad luck" a lot more often than others.
I know this from my own experience.

I really doubt anyone writes their own pseudo-random number generators nowadays when it is part of standard libraries except for really specialized applications. The only limitation of PRNG is that it repeats after a (long) series but you should never see it.

Hardware...possible but its again all standard PC parts.

Go test with /random and do it...say a 1000 times before you start blaming programmers.
