this could be a memory (RAM) issue but also check that your mouse isn't being moved accidentally at the same time. if you have too many peripherals like more than 1 mouse or a mouse+controller plugged in at the same time sometimes the table shaking can cause the PC to have to compute both (makes lag).
otherwise im not sure sorry. only other things i can think of is to make sure game is running admin mode or that theres no other software that is also using your controller. just curious, what operating system are you playing on?

Been playing with controller on PC for 8 years and never experienced a delay. It could be an issue with your controller or driver software.
Unless you're using macros for your skills - which is not recommended.

I have the same issue as OP and tried disabling the microsoft enumerator, but it did not resolve the issue. I have tried with multiple ports on my PC (front and back) and with multiple controllers - PS4, PS5, Xbox 360, Xbox Series X, and Switch Pro Controller. I still get sporadic input lag. This does not occur on other controller supported games.

Does this only happen with Final Fantasy XIV? yes only other games on pc work fine with any of the controllers i listed

Could you try the following trouble shooting?
- Update any controller drivers and make sure its calibrated properly
- Make sure Windows is fully updated
- Try unplugging any other devices and only plugging in the mouse, keyboard and controller
- Uninstall and reinstall any device drivers
- Make sure Final Fantasy XIV is the only program running
