I am having the same exact issue. I've tried all of the suggested troubleshooting actions. I've uninstalled/reinstalled FFXIV, cleared the registry with CCleaner, moved around my USB connections, limited FPS to 60, lowered my graphic settings, used the repair option on the launcher, changed my in game audio settings, uninstalled/reinstalled drivers, updated drivers, updated windows, deleted all my character files, set game to bordered window/borderless window, changed compatibility, ticked Disable Fullscreen Optimizations, ran in DX9 (Game crashed as soon as I was in a highly populated area), disabled firewall, disabled all processes from starting upon rebooting pc, I even tried running as administrator. Nothing is working. FFXIV is the only game I have issues with. Every other game runs like a dream.

I DID NOT START HAVING PROBLEMS UNTIL 6.01.

I will say, however, my FFXIV behaves the same way, with the same exact error code as OP posted. When I launch it, I get the error just as the Rated T for Teen screen is fading out. If I were to move the error out the way and put it elsewhere, I can play the game, but minus the sound. If I were to change the sound settings through my PC during gameplay, it'll immediately lock up and I'm forced to use task manager to close it. I'll post my System Information as well, if it'll help find the cause to this issue, and hope it sheds light on how we can get it fixed. FFXIV is a great game, and the sound is part of the experience, I would love to hear it again.
