Steps:
1. Engage the Terminus Twitcher enemy in the first trash pull after the second boss
2. Enemy will perform the eye attack
3. Trust party members do not turn around to dodge the gaze

Your trust characters are not perfect and will not dodge all mechanics. If it's a non fatal Mechanic they do sometimes eat it. Alisaie is reckless. Urianger is notorious for getting hit.

This might actually be working as intended.
