Changing of monitor causing game to crash and freeze PC?

So i've been playing FFXIV for years now. I'm currently on the Stormblood expansion. I've had multiple monitors, and i've had the same graphics card for a while now with near zero issues with crashes, freezes, frame rate etc. I usually run the game on a standard settings and I always have fps at 59 easily. And once again, no issues.

My recent samsung 24 inch went out, so I was using my old school 17 inch wide screen. No big deal. Well, a friend game me a 24 inch LG 1080p monitor to use for the time being, and now, whenever I play FFXIV, during the game my monitors turn black, sound freezes, and then my PC just sits there. Pretty much a hard crash.

Checked drivers, made sure nothing was running in the background, tried again multiple times, and it still crashes... The ONLY time it DOESN'T crash, is if I change my actual monitor resolution to 720p. Very odd.

Btw, my graphics card is a AND Radeon R9 390. I know it's an old graphics card, but as Ive stated before, ive never had any issues with it. Any help would be great!

    Thanks!
