This exactly. Happens to me and pretty much everyone I know.Audio switching has been an issue since version 2.0 - you need to figure out where you want your sound to come from before you log into the game, and set that up ahead of time.
I've noticed that it really only happens when you remove the sound source entirely, like unplugging a head phone jack or USB dongle. A soft switch, using Windows audio changer, from one already plugged in source to another source, may have better success. I switch from my heavy Void wireless to my light weight blue tooth gym headphones to my permanently plugged in sound bar that way without a crash. If I unplugged the sound bar while I was in the game, though, it'd go poof.
