    I am working on the Alchemist quest "Ultimate Alchemy," for which I need a High-Quality Mega-Potion of Vitality. I have the required ingredients, all High-Quality, yet the chance of crafting High-Quality potions remains 1% before taking any crafting actions, so I am unable to craft High-Quality Mega-Potion of Vitality.

    The ingredients used are no longer available to acquire as HQ, so you are using older ingredients that no longer apply HQ bonus.

    You're a level 52 Alchemist working on a level 45 quest. If you can't HQ a recipe, either get better gear, or use a better crafting rotation, or both.
