Potential Fix:
I used windows start over function and wiped my C: drive then reinstalled all drivers and runtimes before redownloading FFXIV can confirm it's working now with none of the errors above displaying.
Same with me, no response on my forum post so commenting here to keep this thread bumped, also issues since 6.01 when loading up the game i get error and can't play
I used to crash all the time with the dx11 error, and since I was playing a lot of ranked 4v4 Feast pvp this was unacceptable and frustrating since each DC means I lost points.
The only fix that worked for me was downclocking my GPU to 70% power limit with MSI afterburner. I have never had a dx11 error after that. It seems ffxiv does not handle overclocking, or even just regular out-of-the-box clocking well. Note that if you bought a high-end premium GPU, chances are it comes slightly overclocked out of the box.
The tiniest lala.
This just started happening to me after upgrading from 1070 ti to 3060 ti. I can barely play for 30 minutes before Fatal Directx Error 12000002 and it crashes to desktop.
I've tried everything above outside of Underclocking which seems a bit much when every other game I've tried runs perfectly fine.
Sometimes I get DX11 errors when closing the game so I don't really care much.
Still I pray every day that these issues won't suddenly get worse as I've already tried to reinstall my drivers with DDU, run multiple memtests, repair my DX11 install etc...
I'm certainly not gonna underclock my GPU because of some spaghetti code, I'd rather quit the game altogether
Even underclocking I'm crashing. Game is now unplayable and I was just on the final quest of Shadowbringers.. so thats great. This just started happening to me after upgrading from 1070 ti to 3060 ti. I can barely play for 30 minutes before Fatal Directx Error 12000002 and it crashes to desktop.

I also crash teleporting into a city or just switching zones in a city, turning down everything to potato really doesn't work for long, if at all.Not much to add unfortunately but for 2 weeks is happening , 99% of the time happens when i teleport to a city, first was Radz, now Eulmore... So i lose everytime to relog +2h because yes... queue.
Not happening with any other game. And my drivers and OS are all up to date. Nvidia.
Would be nice to see some feedback... I would be happy for a "we acknowledge and tracking the issue"
Here is the error I get it is a pretty big pichttps://imgur.com/lC2ZJnU
Going to hop in here since my attempts at screaming into the void that is the forum hasn't produced any help.
Link to own post including all the proposed troubleshooting that always gets suggested but don't work: https://forum.square-enix.com/ffxiv/...11.exe-crashes
