Since 5.4 my frames have only gotten worse now i get 10 frames while its windowed and it maxes out my gpu with no clue why
My gpu is the Gtx 965 so its not that my pc is horribly under the recommended. Its just so mind numbingly annoying

Looks like it's running hot. Probably need to cool it better.
Guessing this is a laptop, may need to clean the dust out.
What you could do is download a tool to see if something is running too hot, if it's the cpu you might need to change thermal paste (in laptops) or if it is the gpu you might want to check if the fans are all still working, laptops really easily overheat when they are used for gaming. If you have a tower i would say check the vents and also look how your gpu is doing temp wise and if it's getting too hot you might want to buy an additional fan.
But yeah could be anything from faulty hardware to old drivers etc. so in the worst case maybe let a repair shop look into it, they often have more tools to check if something is wrong with your hardware.
Gawd dayum, 86 degrees Celsius is hot as hell, my old laptop with a 1050TI would start lagging at 56 degrees, I think your pc might be overheating a bit.
To be fair really modern gpus can safely reach like 110°C (230°F), 86°C is really hot but still no complete meltdown. So yeah i would definitely check the fans and add some additional ones.
If you have Citrix installed then uninstall it.

Most gpus heavily throttle at those temperatures. Most gpus want to be in the 50-70c range. CPUs can hit 105ish before throttling
86c is considered operating temps for AMD cpus for example. It might be considered high temp for a GPU like Nvidia, but yes, he's getting underclocked due temp.
