Intermittent lag, packet loss, disconnects from Chicago

For the past two weeks, I have had intermittent, sudden bad lag for anywhere between 10-15 seconds to 10 minutes. Actions will take over 3 seconds to respond and I get frequent disconnects, then suddenly everything is fine. Tracert isn't capturing this well because it's so intermittent.

Attached was captured at ~5:30pm CST, 11/23. The lag spike lasted about 15 seconds, then didn't happen again for those 10 minutes.

This is an issue because when it lasts long enough I get disconnected. I've lost crafting materials from disconnecting while crafting. I've disconnected numerous times from duty party content.

Platform (Steam/Mac/Windows/PlayStation): Windows 10
Any error message(s) received? No. Sometimes I will be disconnected without an error code.
Wired or Wireless connection: Wired.
Other programs being used when running FFXIV: Discord, Chrome
When error occurs/How often: Multiple times a day, unpredictably, on and off for two weeks
Techniques used to solve problem: Contacted ISP. No issues were reported on our end.
