Palace of the Dead Luring Trap triggers before even loading into the instance

Date & Time: November 12, 4:14 PM (MST)
    Frequency: Only once (so far)
    World name: Cactuar
    Character name: Gwyn Unlucky
Monster name: Deep Palace Manticore, Deep Palace Keeper
    Class/Level: Machinist 80
    Party or solo: Solo
    In-game time: 11:52 am
Area and coordinates: Palace of the Dead, Floor 141
    Steps:
    1. Begin Palace of the Dead Floor 141-150 (This particular run was also deathless, I was gunning for the Necromancer title)
    2. That's it, that's all I did. Maybe move around in the blue loading circle?

Attached screenshot. Pay close attention to the chat log, where the message for luring trap trigger is displayed before even the message for duty beginning has.

    Wait... where was the trap?
Was your character set to somehow spawn on top of the trap? Or did one elsewhere somehow trigger?

Either way, I'm sorry for your loss.

I believe the monsters had walked from an adjacent room into the starting room and were able to attack before the blue loading circle had disappeared. I'm unsure of the compass direction that room was locationed in, but it implies a distant trap triggered on it's own (unless somehow the trap was in the starting room and the monsters just spawned in an odd location; since we're dealing with a clear bug either way I wouldn't take that possibility off the table)
