I generally commend in the following order:

1 - Someone who made the dungeon fun and entertaining, even if they weren't a great player.
2 - First timers and sprouts.
3 - Someone who was REALLY good at their job.
    4 - DPS
    5 - Healers
    6 - Tanks
7 - Whoever is still left after the rest of the party insta-leaves before I can even click a name.

if im the healer or tank i will give it to the healer or tank. if im a dps i will give it to the other dps. it will be influenced how they play. if they are too bad or somebody was really great.
if its hard to decide i tend to give it to the one who said a greeting. basic manners are in every kind of society important.
i dont look if someone is a sprout or not, they too have to earn it.

For comm criteria in dungeons I use this:

If arrive at last dungeon boss within 10-15 minutes (barring dungeons with intrusive progression such as Bardam's Mettle) comm goes to DPS, tank, healer or no one in that order. If last boss is not reached within 10-15 minutes, no comms.

If I have to decide between 2 DPS I comm the one that didn't die. If neither of them died I comm the one that stayed 2nd on aggro list for the majority of boss fights.

If both DPS don't meet this criteria I go to comm Tank if they did wall to wall pulls while using their cooldowns, or if the healer died and couldn't be rezzed and tank managed to stay alive till boss is killed.

If DPS and Tank don't meet this criteria, I go to comm Healer if they healed via oGCDs and conributed to DPS, but get instant disqualified if I see them casting Cure/Physick/Benefic or if they let people die from unavoidable mechanics such as raidwides (if the DPS/Tanks weren't collecting vuln stacks) or cleansable Dooms.

For Trials/Alliance/Normal Raids I don't give comms because the minimal required contribution per player is so low that you are expected to meet it.

For EX/Savage/Ultimate I go with static, so can't comm.

If they're a BLM they auto get comms because it's my favorite

For real though, when I give out comms to DPS, there's usually a reason.
Tanks/healers almost always get comms, so I try to find good reasons to give dps comms.
I run dungeons a ton, so I know how fast a run typically can go of most dungeons, and if it's not very fast it's either
A: the tank didn't pull very much or
B: DPS are big sucko and don't use aoe/aren't up to gear
If it's the former, there's almost no way in hell that I'm going to give the tank the comm, especially if it isn't their first time or aren't a new tank.

Giving comms to healer is different, it's easy to tell if they're doing a bad job. But it's also very difficult to tell if they're doing a good job too, unless you're looking for it.
If they're a godlike healer, they're probably going to get comms, but those are hard to find because you either get a clean run where nothing goes wrong, or a really bad healer.
Because of that, I typically don't give healer comms unless I've seen them do something.

DPS typically get my coms if I'm running dungeons with my friend who uses an... abacus to calculate damage and they're top DPS.
However, if either DPS fails to get top damage, I fall to either Tank/Healer because then you have a DPS that is big sucko.

Although, if there are no real good identifiers, BLM will 100% get my comm if there's one there

Hmm, usually it comes down to: Pure randomness, I just decided to give it to someone else aside from healer, or the DPS made something that highlighted them, like killing the boss after everyone else died or were being helpful to a new player in a dungeon that is struggling a bit with mechanics, etc.

I tend to give Commendations to DPS when I actually notice how well they're doing.

Tanks and healers get coddled enough as it. Unless they pull wall to wall and the healer is doing aoe non stop, the dps who suffered with me gets the comm.

Things died super fast: Give to DPS (will pay attention to who tops threat list)
Things were pulled very efficiently and faced away from the party, interrupts and defensive cooldowns used: Give to Tank
Heals were on point (uses heals when needed, not just to top off) and shields and regens maintained while healer DPS's: Give to Healer

tie between two of these things: give to whomever has the funniest name or coolest reference in their name.

all of the above happens: Has never happened.

EDIT: Also will give if you make me actually laugh, save the run through skillful play, or show actual enthusiasm for the game and not for optimizing.

Sometimes I'll give it if I really like someone's name/glam. Or if they are friendly and talk or help in a significant way.

If they are a Lalafell and respond cutely to my headpats.
    If they are a sprout.
If they are last to leave.

Often I just randomly give commends though, haha.
