Really bad lag issues on the PS4

Hey!

For about a week or so, I've been having really bad issues with lag.
NPC's dialogue pop-ups take about 30s to a full minute to open; mob, NPCs and other players get completely frozen for up to 5 minutes and I get random disconnects.

Any suggestions?

I'm on the PS4.
