Tank never loaded into dungeon, unable to remove from party

Last night I queued with one person into a leveling roulette, and we were placed in The Stone Vigil. Our tank never connected or appeared, and after a long wait in which we discussed whether the tank DCed while loading, the starting ring disappeared. The tank still not not appear. I suggested to the group that we kick the tank so that we might find another. Once that was done, the game text stated "A party member has been dismissed" but the tank still appeared in the party list. There was no way to continue, so I suggested that we leave. The DPS we were matched with had waited half an hour for the dungeon, so I said I would invite them and we could queue to quickly get back into the dungeon they needed. Unfortunatly, we did not vote abandon because 15 minutes hadn't passed, and we assumed because the game had stated the tank had left, we would not get a penalty. Unfortunatly, the DPS did get a penalty.
