    I've got it working using SDL2 Gamepad Tool <https://generalarcade.com/gamepadtool/>.

    SDL2 Gamepad Tool is very simple tool, no need for install, it is standalone utility app which only helps creating gamepad mapping.

    Steps
    - Quit FF14
    - Connect PS5 controller
    - Open SDL2 Gamepad Tool
    - Confirm the controller is working
    - Click "Set Mapping As Environment Variable" button
    - Start FF14

    Now it should work, and also button calibration in system config works as well.

    FF14 is using SDL2 for gamepad mappings, and we can adjust the mapping by setting the environment variable. I assume we must wait for FF14 to officially update SDL2 lib or add mappings for PS5 controller. They've done this for PS4 controller, so I think someday, it would happen.

    By the way, I couldn't find a way to use touch pad button on PS5 controller. Share button and Start button works, but seems like SDL2 Gamepad Tool is not capable of generating the pad button mapping. At the moment, you can use Share button instead of the pad button.

    PS5 controller - Not Working - doesn't work/map using this method.
    Razor controller - Working - was able to remap using this method.

    Brilliant, this seems to work for the Xbox Series X controller (at first glance) on macOS Monterey. Thanks for sharing this!

    Issue still exists

    I still can't get any controllers working with FFXIV. They are working fine with other games though.

    macOS 12.2.1
    MacBook Pro 12" 2021
    M1 Max
    64GB

    The issue happens with at least these controllers:

    PS4 Controller
    PS5 Controller
    Xbox Series X Controller
    Xbox One X Controller
    Steam Controller

    And I am unable to use any of these controllers both via bluetooth as well as via USB connection.
    The game is not launched via Steam as I can't log-in to the steam version of the game with my existing account.

    The Game correctly displays my Controller in the System Configuration Window, but no Inputs are working, not even during calibration.

    I do apologize for this ongoing issue regarding the PlayStation 5 DualSense controller not properly mapping in Final Fantasy XIV.

    Just to gather more information, is the controller fully synced via Bluetooth or wired cable to the Macbook Pro?
    Has the other controller given you a problem for Final Fantasy XIV?
    Have you fully updated the drivers for the controller(s)?
    Are you able to use the PS5 DualSense Controller for other Mac games?
    Lastly, have you found success in playing Final Fantasy XIV with a PlayStation 4 DualShock controller?

    I've tested both, and both results in the same result. By the way, I'm using iMac, but I'm pretty sure it's same for the MacBook Pro.

    I've tested with PS4 controller and it's working fine.

    Yes.

    FWIW, the controller issue is likely an issue with Crossover (the system that wraps the Windows client to run on macOS); Codeweavers—the folks who write Crossover—just released Crossover v21 last month, and it included a number of fixes specifically relating to Xbox Elite Series 2, Xbox Series X, and Sony DualSense controllers. (Major fixes to Bluetooth connectivity issues, and minor ones to input mapping.)

    So, hopefully the problem will go away whenever FFXIV bundles an update using the newer Crossover.
