Steps: I was in Ul'dah, happily crafting away, chopping through the log, file file file chop chop chop... when some guy I didn't know from Adam started asking me to be his friend. I ignored him (just regular I'm-busy-here ignore). He kept asking. I kept ignoring. He eventually sent me a friend request. I can't deny it. It's supposed to go away after a week but I've had it for months, I even took a 40-day break and it's still there.
I did /ignore him at some point as well, this may be relevant. Maybe the friend notification doesn't go away properly if it's from someone in your blacklist?

To decline a friend request, you can click the notification on the right of the screen. Decline it, then it will ask you if you are sure you want to decline it. Click Yes.

Try opening your friend list so that it updates the status of your friends properly, including the status of accepted or declined.
