Could you go into Discord and make sure that the Discord Overlay button for Discord is red.

Cog Wheel (Bottom Left) > Activity Status > Final Fantasy XIV > Monitor Icon on right is Red.

Also, is it ONLY a DX11 error or is it the ffxiv_dx11.exe thats crashing?
Yeah I've made sure the icon is red for the discord overlay, here's the error that I usually get:
