It's just RNG.
Every time you get a loot opportunity, an invisible roll is thrown and you have x chance in 100 to get it. The rate has increased since the last patch or is it just an impression.
If the chance of loot is for example 0,01% you have a lot of chance not to get what you want.

The notion of grind for me is just very low gain, but at a good ratio, like the Eureka light farm from Stormblood on sleeping dragons. Or farm xp on mobs packs in loop.

I think the loot rate has increased, since the price per MB has gone from 15M to 400k for hairstyle. But it's still RNG.
