Frequent Crashes/Freezes

EDIT: I am not sure how I can receive notifications from this post, but I will try to check this frequently.

I've been having this issue since last year mid-2020. I would be able to play for ~30 minutes/longer and then my game would freeze/crash. If on Dx11, I'll get a black screen iirc. If on Dx9, my screen will freeze, rarely, it will resolve itself. During such freezes, I can still hear the audio and alt-tab.

Recently, I fixed my VCore to 1.28 instead of Auto. Now, when I crash, my whole PC starts lagging out and I have to force shut-down. This makes me think it could be partly a CPU issue? I will try reverting to the original VCore settings.

A few of my games also suffer from a similar issue - some of them crash entirely. I do not think lowering the graphics affects it very much.

This is extremely frustrating. My GPU can run games at high performance, but it seems to crash frequently too. This is despite my underclocking/limiting power to GPU.

Attempted:
1. Clean installing the latest NVIDIA driver
2. Clean installing FF14
3. Clean installing OS
4. Underclocking GPU Core Clock / Memory Clock by 60 MHz/ 100 MHz respectively
5. Playing on Dx9
6. Disabling overlays (Game Bar / Steam / Discord)
7. Playing without Chrome in the background
8. Playing with Fast Boot disabled.
