i`ve clean restall,
rebooted
reopen steam
changed install location on clean install
reinstalled video card drivers, and game,
windows format
pray to gods and demons
blood sacrifice
blood pact,

none of the above seemed to work, the only correlation it has is that the less ALT+TAB i use harder it is to occur, but that is terrible if im trying to stream and have to monitors... help please!
