Not Square-Enix support, but... out of curiosity, which audio output are you using? It looks like you have three: the audio output that accompanies video output from your Radeon, your headset (using the default Windows USB audio driver), and the Realtek HD sound presumably on your motherboard itself. And given that the crash is in XAudio 2.9, I half-suspect something is wonky with one of the audio drivers. (My gut feeling is that the XAudio initialization is failing -- hence crash dialog #1 -- and then FFXIV is not handling that failure very well, leading to crash dialog #2.)

If it's the Radeon one, I'd consider ripping out the AMD drivers entirely and reinstalling Catalyst clean just to see if something's cached a little bit wonky. If it's the Realtek one, I'd see about installing Realtek's own drivers rather than the ones built-in to Windows, just to see if that fixes anything; I've had problems with the Windows Realtek drivers myself at one point.
