Frequency: Same cutscene, different times 3 times already. 2 times before fixing, 1 time after. (it seems to only crash during the cutscene sections, not the fight sections in between.)

Steps:
1. Cutscenes play, bsod's entire system during random parts of the cutscenes

Fix steps
1. Uninstalled GPU drivers using DDU in safe mode (after getting a failed install on the newest drivers through geforce experience)
2. installed latest GPU drivers without being connected to the internet.
3. Gave CPU more voltage headroom regardless of previous stability. (changed from -0.125V to -0.11875V)

I found a 3 year old post describing the issue I am experiencing from u/frustrated_tech on this subreddit.

To put it short.
"Display driver nvlddmkm stopped responding and has successfully recovered."

This is the error I am receiving.

It has crashed during 3 seperate occasions during the cutscene, 2 times before fixing and 1 time after.

I have never experienced this issue before, not with FFXIV, not with any other application.

How I have tried to fix the issue:

Change cpu voltage offset from -0.125 to -0.11875 to give it a little more headroom (though that should not have mattered in the first place as its a gpu error, and the system has been stable on this offset for forever, I thought it was worth trying.)

Update GPU drivers ( the driver update failed which lead to my next step)
Fully uninstalling display drivers using DDU in safe mode. After this, without being connected to the internet, installing the latest driver unto my system that I could not download before as the download got an error and failed.

Check for Windows updates (system already up to date)

To Clarify:

Non of my components are overclocked, only undervolted with 100% stable settings, every other setting is stock. My RAM is just running its regular old DOCP profile as well.

2080 Ti Founders Edition ( Stock clocks, 925mV) New driver version 461.40

Ryzen 7 5800x (stock clocks, PBO Disabled, Voltage offset of - 0.125 (now - 0.11875))

G.Skill Trident Z RGB 2x8GB @ 3600mhz on standard D.O.C.P.

My CPU and GPU are on a full custom loop with a 360 and a 240 rad, GPU not exceeding mid 50's and CPU not exceeding mid 70's (yes, idk, AMD gets hot). So overheating is definitely not the issue.

As mentioned before, this is the first time I am ever receiving this error, and I have been able to replicate it during 3 different times of the same cutscene but also after different sections of the fights in between.

The only other error I have received multiple times was simply the game crashing with a DX11 error, which was just fixed by relaunching the game. While annoying, at least I was still able to continue playing, the current issue black screens my system. ongoing dialogue will finish playing, music will be the only thing left after that. After which a restart of my system is required.

Has anybody recently experienced the same issue and knows how to fix it per chance?

So to recap.
Game ran fine before this cutscene, even the longer ones never had any issues.
Everything is running stock clocks, so OC's are not the issue.
Game is running at locked 165fps, dips under it at some points though cutscenes are fine.
Heat is not the issue as I am running a custom loop.
Is my hardware just too good for the game or something? Feel kind of torn on this issue.

The game crashes on my laptop as well, and I have noticed it also crashing in the last cutscene of 3.55. (The game is installed on my C drive on my laptop so the drive does not make the difference.)

That shouldn't be on a PC, specifically a computer that doesn't have an Intel based CPU with built in GPU and a discrete nVidia GPU.
That driver should only be on a Intel CPU/GPU laptop, with a built in discrete nVidia GPU.
That is a driver made to switch the display rendering between the Intel GPU and nVidia GPU on the fly.

Is it possible you have your laptop and PC both synced via Windows 10 Sync Settings?
If so, turn that off.... that is bad.

I usually take care in turning all of the sync settings off. I will check if anything is still on.

So this is kinda funny. There is no trace of optimus anywhere in my files. I've tried looking up file locations and everything online but I can't find anything on my computer. Secondly, there is no other drivers or programs listed in the system specs that sync with my laptop. And lastly, I checked my nvidia control panel and everything is just set to use my 2080Ti as there is no option for an iGPU in the first place. Really weird.

I am no longer using shadowplay in hopes it would fix the issue.
The game just crashed with a similar error though it did not crash my entire system. the game referred to it as a fatal DX11 error and event viewer shows the same error as listed in the original post.
what might have lead to my computer not blackscreening however is the fact I only have a single display on, as my second display is a different resolution, it might be the cause for the blackscreens upon crashes.

Welp, another update to the thread.

Driver 456.71 being a driver which a friend of mine is running who never has any issues with her game crashing at all.

The game crashed today during the first 10 seconds of a cutscene while just having switched back to driver 460.89 (the last driver to not have any crashes on my system as far as I was aware)

After this I went down to 456.71, was able to finish the cutscene, and then a single second into the cutscene of the mission right after, it crashed again.

On top of going through all these driver versions, I have also refrained from launching my game through steam, and disabled shadowplay. I am running the raw mogstation downloaded launcher, no overlays, no applications interacting with the game or anything, and it still crashes.

It is actually becoming so game breaking that the game is unplayable, and every single time I go into a cutscene or load into a new area by whatever means, I'm there anxiously crossing my fingers for my game to just not crash.

During Post ARR content, around the time I updated to driver version 461.09 was when I first started seeing DirectX11 crashes. This was an error that showed "an unexpected error" with a couple of DX11 lines.
This error has only occurred about 2 times in the past couple of days, and instead since late post Heavensward and now throughout Stormblood, any type of cutscene or loading screen has a chance to cause a fatal DX11 error, which for some reason black screens my screens with no way to interact with anything anymore and calls for a restart of my system every single time.

No other game or application on my system suffers from this issue. FFXIV is a first and the only to suffer any kind of consistent DX crashes.

This is a serious issue. I literally just resubbed my account a couple days back and the game is now in a state where I can basically not play it at all.
I have seen several threads on reddit and this forum, as well as some other small places of countless of people experiencing the same issue, and never have I seen a concrete fix for the issue before the threads got closed because inactivity due to the age of the thread.

Could you try using a registry cleaner such as CC cleaner? It would run through the PC and remove files that may have been left behind by installations and other activity on the PC.
