Cannot Progress NG+: Shadowbringers Chapter 1

Steps:
1. Progress story to quest 'Warrior of Darkness'
2. Proceed to step 'Speak with Glynard'
3. Upon speaking with Glynard receive 'Unable to Commence Cutscene' error.

I had completed all role quests, role master quests, and void quests outside of NG+ prior to having this happen. Additionally, the NPC Cerrig (Crystarium - The Wandering Stairs (11.2-15.2)) has a double of himself in the exact same place when NG+ is enabled. If you click him, he provides the post-Void Quest dialogue, but as he moves, the double of his NPC stands there and does not move. This double NPC is not there when NG+ is not enabled.

Additionally, Unukalhai continues to be in the Wandering Stairs upon completion of the Void Quests, even with NG+ enabled, despite the fact that chronologically he should not be there at that point in the story if NG+ is enabled.
