Constant DirectX Errors and Crashing, Game is Unplayable

I've been having this issue for about three months or so now, not sure what's causing it. Sometimes it happens after 5 minutes, sometimes after 2 hours, sometimes not at all. But when it does, my game randomly freezes up and crashes and I get the 11000002 error and I basically have to restart my computer.

So far, I have tried:
- Updating Windows 10
- Updating my drivers
- Reinstalling the game
- Changing resolutions
- Reinstalling graphics drivers
- Factory resetting my PC
- Disabling Discord Overlays
- Playing on DX9 instead of 11 (the game won't even launch for me if I try that, I get a separate error)

And probably a couple things I've forgotten. I feel like there's nothing left I can try. ):

Does the crash occur in a specific location in game? Additionally since the crash started happening, were any changes made to the PC?

I've had the issues occur pretty much everywhere, from the summoning bell outside my house, to busy cities, to Bozja, etc. It doesn't seem to be affected by the amount of people nearby from what I've seen.

Can you elaborate what you mean by "changes"? The only things I have deliberately done that I can think of are to install Windows Updates as they have been rolled out, tried to keep my drivers up to date, etc. I have not installed any new hardware or anything of that nature.

Does the crash occur if the game is run on windowed mode or borderless full screen? Additionally when the crash occurs, is anything over heating and do you have a way of monitoring these temperatures ?

It's happened in both windowed mode and borderless full screen. I don't believe anything is overheating, especially in the instances when I reboot my computer, start up the game, and it crashes immediately. I do have a program on my computer that manages my GPU and CPU temperatures.

I've tried running the game with very few programs open (especially Google Chrome) to try to prevent these issues as well but its usefulness has seemed hit or miss.

Yeah, I browsed through a lot of those threads before posting mine and trying any solutions I saw suggested to no avail. ):
Thought maybe there was something else causing my issues that others weren't experiencing.
