Reently (10/17/2020) I've been unable to play in fullscreen mode because the framerate is dropping to 3fps at login and remaining that way unless I switch to windowed mode.
Additionally running in borderless windowed mode creates the same effect.
While running in windowed mode I'm only getting about 30fps.

I think you're using a laptop based off your cpu and gpu? This is going to seem super basic, but it's happened to me once before and took me an embarassingly long time to figure out. Has your power cord come loose a bit? If the cord isn't properly plugged in, laptops will usually force using the on-board graphics card rather than your discrete graphics card in order to conserve power, and that results in abysmal frame rates since the on-board graphics cards just aren't equipped to handle any sort of 3D gaming.

Very likely due to a Windows or nVidia update, the computer has switch FF14 to run on the Intel HD Graphics system on the CPU instead of the nVidia GPU.

As far as I can tell it has been switched back to the nvidia card but with no change in performance. I'm still running into the same issue. I followed the fix in the link above.

Good morning and thank you for posting on the Final Fantasy XIV Technical Support Forums.
Have any changes happened to the device before the issue began? We would also like to recommend several other troubleshooting steps:
1) Make sure that your hardware is not overheating and remove any overclocks you may have on your graphics card or processor. Please check that any fans or vents are clear of blocks.
2) Disable any additional programs that could cause interference with the game itself including chat programs. Also, remove any custom configurations made using graphics driver-level programs
3) Remove any mods or injectors from the game or system
4) Turn off Fullscreen Optimizations. Find the executable file for the game (normally found in the install folder), right-click it and select 'Properties'. Under the 'Compatibility' tab, check the box labeled 'Disable Fullscreen Optimizations', then click 'Apply'.
5) Remove any unnecessary USB or audio devices (controllers, headphones, flash drives etc.)
6) Update and reinstall the drivers for your graphics card
7) Perform a clean boot by opening the task manager and select the 'Start up' Tab. Disable everything
