I have recently started playing the game so it is a fresh download of the client.

When I was having the crashes I checked on will it run and I met the recommend requirements.
Your issue of stuff not loading in when changing maps points to one of two things -
A: slightly faulty download of the game (this can happen sometimes, re-download could fix this)
B: a faulty storage device on which the game is held (i.e. an SSD on its last legs, or a slightly faulty hard drive) <-- this is the least likely of the two
C: (the least likely of all, ever) faulty RAM, although if its only FFXIV that experiences these issues, I doubt its this.

My advice: Re-download the game once more (with no interruptions in the download process - i.e. shutting down the launcher mid download, if you can).

Edit 2: It's a very minor possibility, but - lock your in-game framerate. Just in case the graphics card is experiencing overheating. The more framerate the graphics card feeds (if unlocked) the hotter the card will get. Oftentimes running at high framerate at lower settings - puts more strain on the card, than running at max settings and low framerates would.

P.S. I know this post and the quoted text does not belong in a disconnects thing, but with my suggestions the above issue should be resolved.
