I'm all for QoL improvements, the problem here is that what OP is asking wouldn't even work.

A trigger to a panic or anxiety attack is not to be taken lightly, it is an emotional reaction to something traumatic that has happened in the past. Let me be clear here, a simple warning would do nothing to stop the trigger, this is a body response to what is happening in front of you.

So what can SE do?
If we are talking about an opt in, then SE could just remove the scenes altogether. However this causes a few problems
1. You lose immersion and context, you are likely to not know what is going on as the story is so interwoven.
2. Where would SE even start? You would need multiple opt ins for every situation, not everyone gets triggered by the same thing, everyone's anxiety/ panic attacks aren't the same. Even if SE did multiple opt ins its likely they will still get it wrong due to how impossible the task is.

It is a nice idea, however it is impossible to implement and it's not healthy to play something that triggers you.
