The hat disables the ears on Mi'qote, which makes me wonder why they didn't just do that for Viera. It'd be ridiculous, but it's still better than nothing.

But your complaint isn't invalid, we Miqo'te have a lot of hats with less headroom that will either give us ear-socks, or just holes to fit our ears through.

As for why viera don't just have holes though, there is a reason for that too. The three ear options are not the same size or in the same position, and so they can't just put a different texture on that has holes, because there would actually need to be three of them, and I suspect that even if they wanted to do that the game probably has no way of differentiating characters based on the ear type they chose like it does for race or clan or whatever.
There is still another solution which tons of people have pointed out though - they could just not do anything and have the ears clip out the top, just like how miqo'te tails clip out the back of most gear without any alterations to the model. Not only would we all just get the basic idea that there are holes even though they're not drawn on there, but it's not like they seem to care much about things clipping through each other elsewhere in the game.

The fact that the hat is smooth on top really makes me question why the ears don't just poke out like they do on some hoods
