I guess, I always viewed social engineering attacks more as a scam or a con. Overall I just cannot fathom how people are so gullible, but I have heard some horror stories on this form and from people that work in IT how easily tricked people can be. Still shocking.
pretty much a phishing scam is the bait, hence phishing (fishing) the hack is them using key-logging/ data recording to get a hold of your info through the phishing attempt.
phishing and key logging has existed since the aol days. And schools used to have a computers class to teach computing, and internet safety such as not clicking on links from ppl.
shame this is no longer taught since ppl get scammed more and more now.
(ps im 35 years olds, and majored in graphic arts in my vocational high school web design was part of it and phishing/ site security was part of it)

As others said use a one time password it fixes all the problems besides phishing sites. And for those... never ever go to any link you ever see in a mmo unless it is something directly from a server message or the games official website/twitter ect.
In terms of guild banks being stolen. As inconvenient as it is this is why you should not store valuables in 1 place where 1 person has unlimited rights to take them. Guild banks are good just not ones where everyone has full access to everything. Best to split up the bank among officers/trusted members. That way if something does happen you lose say 1/5th of everything instead of everything.
Not just in FFXIV but any mmo. It just opens you up to the following issues if you do not.
1) Someone gets hacked or scammed out of their account and stuff is stolen
2) Member/Officer gets disgruntled and takes the whole bank and bails
3) Leader gets greedy and uses it all for themselves and friends
I seen all three of these in my time playing mmo between EQ, FFXI, WoW, Archeage, and FFXIV.
It boils down to, we need the ability to limit how much of what slot you can take, like "anybody can take 200 crystals per day" or "take up to 250k gil" etc... id really love to keep ours more open but the risk is just too high >_>
This, better settings to control our bank will be really appreciated.
They're working on it.

Hopefully they add what WoW did, an option to have a 2FA requirement for characters to be assigned certain ranks with more sensitive access to the guild settings and bank.
Still, in the meantime the best thing is for players to be smart in the first place. Don't fall for the scam shouts and tells. Be cautious of what ranks and permissions you give FC members. Even someone cautious and trustworthy can end up getting compromised if they're sharing their device with other users who aren't so cautious (happened to one of our FC members that allows her daughter to use her computer).
